JLC Transfer Request Form

Please completely fill out the following form to submit a transfer of membership from the Junior League of Chicago to another Junior League Chapter. If you are looking to transfer TO the Junior League of Chicago from another Junior League Chapter, you must request a transfer of your membership from that Junior League Chapter. If you are looking to reinstate your membership from a resigned/inactive status, please fill out the reinstatement form.
